A 68-year-old man with atrial fibrillation on warfarin (INR stable at 2.5) was started on amiodarone 3 weeks ago. He now reports gum bleeding when brushing. BP 132/80, HR 72. Hemoglobin is unchanged. INR is 6.8.
What is the next best step in management?
Show AnswerHide answer
Hold warfarin and give low-dose oral vitamin K. Amiodarone inhibits CYP2C9, raising warfarin levels. Minor bleeding, no FFP or PCC needed.
Order “discontinue warfarin,” “vitamin K, oral,” and “INR” together, then advance the clock 24 hours to recheck. Restart at a lower dose before you close the case.

Keep doing UWorld and NBME blocks the way you already do. Flag a miss in one click and openHyGuru turns it into a vignette flashcard, matched to Dr. Damania’s teaching.

What residents and students say
4.9
“Rahul teaches you how the question is built, not just the facts behind it. Every case turned into a framework I could use on the next vignette.”
“Rahul is not just another ‘Kaplan type’ company, he is someone that uses this information in his daily career, in addition to being an excellent teacher.”
“The way the material is organized is unlike any other review source I have used. Rahul makes all his teaching centered around active recall.”
Rahul Damania, MD, MS Ed
I’m a pediatric critical care physician at Cleveland Clinic Children’s, Faculty Editor for First Aid for the USMLE Step 1 (2026), and the founder of HyGuru.
Every day in the PICU, I watch residents make the exact decisions Step 3 tests. My approach is the one you may remember from Step 1 and Step 2: vignette-based, NBME-focused, and built on question strategy. No fluff.
Questions
What are the logistics?
Three live sessions on Zoom: Friday, Oct 23 (2–5 PM ET), and Saturday–Sunday, Oct 24–25 (10 AM–4 PM ET).
How much is it?
$479, one payment. That covers all 3 live days, the pre-recorded library, the flashcards and openQbank Connector, and both guides, with 3 months of access.
I’m on service that weekend. Will I get the recording?
Yes. Every live session is recorded and posted to your dashboard, so you can watch on your schedule.
What does the openQbank Connector actually do?
The openQbank Connector works inside UWorld and NBME practice exams, on both Chrome and Safari. Flag a miss in one click without breaking your block, and it becomes a vignette-based flashcard for review. It’s built and run by openQbank, and specially tuned for openHyGuru users.
Is this a full Step 3 course?
No. It’s a focused review of the concepts most likely to cost you points. Use it alongside your qbank, not instead of it.
How is this different from what I used for Step 1 and Step 2?
Same method: active recall, integration, and isolating the high-yield concept. The difference is the vignettes, which are now about management decisions you make on the wards.
To the residents reading this:
Many of you studied with HyGuru for Step 1 or Step 2. Some of you are back now, at 11 PM, after a 14-hour day.
That isn’t lost on me. I built this review to respect your time: no filler, only what moves the needle on test day.
I’ll see you on October 23.
— Rahul
